Pas Foto 2×3 2 Lembar Untuk SKCK, diurus di Polsek 1. Foto Copy Surat keterangan dari RT/RW/Kelurahan/Kecamatan setempat 1 Lembar 2. Foto Copy KK (Kartu Keluarga) 3. Foto Copy KTP 4. Pas Foto Berwarna 2 Lembar Tambahan SKCK untuk CPNS di Polrestabes Surabaya Jl. Sikatan 1. Surat Pengantar dari Polsek 2. Pas Foto 5 lembar (hasil foto di studio bukan foto editan, hasil scan-an, atau yang lain dengan kualitas gambar buruk). 3. Mengisi Formulir yang telah disediakan.
